Care begins with regular, reviews, updates and communication.
We’ll need the client’s location, clinical and support needs, known risks, current care arrangements, funding status and preferred timescale. Any relevant assessments, care plans or professional reports will also help our clinical team review the referral.
We support adults and children with a range of complex and long-term health needs. These can include ventilation and tracheostomy care, respiratory support, enteral and PEG feeding, seizure management, brain and spinal injuries, and neurological conditions. Every referral is assessed individually.
Mobilisation timescales depend on the client’s needs, location, funding, recruitment requirements and specialist training. Once we have reviewed the referral and completed the clinical assessment, we will explain what is achievable and provide clear updates throughout.
We recruit a dedicated team around the client’s clinical needs, routines, preferences and goals. Care professionals complete safer recruitment checks, specialist training and competency assessments before delivering support.
You’ll have a named point of contact and agreed communication throughout mobilisation and ongoing delivery. We can provide progress updates, care reviews and relevant reporting based on the client’s needs and agreed arrangements.
We can support packages funded through NHS Continuing Healthcare, Children’s Continuing Care, Personal Health Budgets, Local Authority or joint funding, as well as private, insurance and compensation arrangements.
