Neuromuscular Care at Home

Specialist, nurse-led care at home for MND, muscular dystrophy and spinal muscular atrophy. Individual assessments guide care team training and ITU nurse input where needed.

Neuromuscular, Home Care

Neuromuscular conditions affect people differently. Individual assessments, with input from experienced ITU nurses where needed, help determine the support required at home. Care plans reflect each person’s mobility, respiratory needs, daily routines and goals.

Care may include mobility, transfers, fatigue, personal care, medication, breathing and overnight support. Dedicated teams receive face-to-face, in-house training tailored to each person.

When moving around takes more effort

Neuromuscular conditions can affect muscle strength and movement. Walking, changing position, reaching or using your hands may become harder, although the impact varies from person to person.

  • Getting around or transferring may require help or specialist equipment.
  • Changes in hand or arm strength can affect everyday activities.
  • Mobility aids and positioning support may become part of a routine.

Independence includes having choice and control,
even when you need support.

Care begins

Care begins with regular reviews, updates and communication.

We provide personalised care for people with motor neurone disease (MND), muscular dystrophy and spinal muscular atrophy (SMA), subject to assessment. Support can include personal care, mobility, nutrition, communication and respiratory or ventilation needs. Our ITU nurses provide clinical oversight and regular nurse visits, working alongside the person’s specialist team.

Yes, where assessment confirms their needs can be safely met. We tailor care around the person’s energy levels, preferences and daily routines, helping maintain safety, comfort and independence. Our ITU nurses provide clinical oversight and coordinate with relevant specialists about breathing, swallowing, mobility and communication support. Our ITU nurses complete individual risk assessments covering mobility, fatigue, swallowing, breathing and equipment use where relevant.

Options can include NHS Continuing Healthcare for adults, Children’s Continuing Care, local authority or jointly funded packages, Personal Health Budgets and private funding. Eligibility depends on individual needs and relevant assessments. Our team can discuss funding routes, while our ITU nurses assess the support required and provide clinical oversight of the agreed package.

Our ITU nurses provide face-to-face, in-house training and regular training in the client’s home, using their own equipment. Depending on the care plan, this can include ventilation and airway equipment, feeding equipment, mobility aids and communication aids. Competency assessments and ongoing clinical oversight help staff provide care that reflects the person’s needs and specialist guidance.

Our ITU nurses assess needs and provide ongoing clinical oversight through regular visits and reviews. We work with the person, their family and specialists to update care plans, equipment and staff training as requirements change. Waking-night or 24-hour support can be considered where assessed and agreed, with the person’s choices central to planning. On-call clinical support is available to the care team for concerns arising between visits, with escalation guided by the individual’s care plan. Risk assessments are reviewed when needs or circumstances change.

Complete our online referral form or speak to our team about the person’s diagnosis, current support, equipment and specialist care plans. Our ITU nurses assess suitability, complete risk assessments and plan ongoing clinical oversight. We then prepare a tailored package, with start dates dependent on funding, equipment and a team trained for the individual’s needs.
