Neurological Care at Home
Nurse-led neurological care at home is based on individual assessments, with ITU nurse input where needed. Dedicated care teams receive face-to-face, in-house training tailored to each person.
Neurological, Home Care
Neurological conditions affect people differently, and care requirements can vary. Individual assessments, with input from experienced ITU nurses where clinically required, help determine the level of support needed at home. Care reflects each person’s health needs, mobility, communication, routines and preferences.
Care may include support with mobility and transfers, personal care, medication, fatigue management and overnight care. Dedicated care teams receive face-to-face, in-house training tailored to individual needs, including complex clinical care where required. Care plans are reviewed as needs change.

Understanding Neurological Conditions and the Care You May Need
Neurological conditions affect everyone differently. Select an area to explore how they may affect daily life and what care and support may be needed at home.

When familiar movements feel different
Depending on your condition, you may experience changes in strength, balance, sensation or coordination. Everyday tasks may need more time, equipment or assistance.
- Walking or transferring may require support.
- Hand movements can affect dressing and practical tasks.
- Your mobility needs may vary with your condition and activity.

Care begins
Care begins with regular reviews, updates and communication.
We support people with complex neurological conditions and associated health needs. Depending on assessment, care can include personal care, mobility, medication support, nutrition, communication and respiratory or feeding support. Our ITU nurses provide clinical oversight and regular visits, with a care plan that reflects the person’s condition, routines and priorities.
Our ITU nurses deliver face-to-face, in-house training and regular training in the client’s home using equipment relevant to their care. Training is tailored to the person’s mobility, communication, medication, feeding and respiratory needs. Ongoing clinical oversight and competency reviews help the team follow the care plan and recognise changes that need escalation.
Potential routes include NHS Continuing Healthcare for adults, Children’s Continuing Care, local authority or joint funding, Personal Health Budgets and private funding. Eligibility is based on assessed needs and the relevant funding criteria. Our team can discuss the next steps, while our ITU nurses assess clinical requirements and provide oversight of the agreed care.
Yes, where individual assessment confirms the person’s needs can be safely met at home. Support is built around daily routines, comfort and independence. Our ITU nurses provide clinical oversight and risk assessments, working with neurological and community teams to review mobility, swallowing, medication, respiratory needs and changes in health.
Our ITU nurses assess support requirements and provide ongoing clinical oversight through regular visits and reviews. We update the care plan with the person, their family and relevant specialists as needs change. Support can include waking-night or 24-hour care where clinically assessed and agreed, with clear plans for raising concerns. On-call clinical support is available to the care team for concerns arising between visits, with escalation guided by the individual’s care plan. Risk assessments are reviewed when needs or circumstances change. We will then update our staff for training.
Complete our online referral form or contact us with details of the condition, current support and any specialist care plans. Our ITU nurses assess whether we can meet the person’s needs and establish clinical oversight. We then coordinate staffing, training and equipment, with a start date dependent on these arrangements and funding.
