Learning Disabilities & Complex Needs Care at Home

Nurse-led care at home for people with learning disabilities, autism and other complex conditions. Support may include one-to-one care, help with daily routines and assessed clinical needs.

PRACTICAL SUPPORT. PERSONAL CHOICES.

Learning Disabilities & Complex Needs, Home Care

People with learning disabilities, autism and other complex conditions can have very different care requirements. Individual assessments, with input from experienced ITU nurses where clinically required, help establish the level of practical and clinical support needed at home.

Care may range from one-to-one support with communication, personal care and daily routines to staffed teams supporting complex health needs, medication and overnight care. Care teams receive face-to-face, in-house training tailored to individual requirements, with care plans regularly reviewed as needs change.

01 / 04 · Communication & understanding

Ways to communicate

People with learning disabilities, autism or other complex conditions may communicate differently or need information presented in accessible ways. Support starts with listening.

  • Speech, signs, pictures or communication devices may be helpful.
  • Information may be easier to understand in short, familiar steps.
  • Preferences, expressions and everyday communication should be taken seriously.

Independence includes having choice and control, even when you need support.

Care begins

Care begins with regular reviews, updates and communication.

We provide personalised support for people with learning disabilities who also have complex care needs. Care can include personal care, communication, daily routines and associated health needs such as epilepsy, feeding or respiratory support. Our ITU nurses provide clinical oversight and regular visits, with input from relevant learning disability and other specialists where needed.

We take time to understand the person’s communication style, preferences, abilities and what helps them feel comfortable. Our teams use accessible communication and familiar routines to support choice and participation. Our ITU nurses provide clinical oversight of health needs, while involving the person and, where appropriate, their family, advocates and specialist professionals in care planning. Our ITU nurses complete individual risk assessments covering communication, health needs, daily activities and environmental or sensory factors where relevant.

Funding options may include local authority or jointly funded packages, NHS Continuing Healthcare for eligible adults, Children’s Continuing Care, Personal Health Budgets and private funding. Eligibility depends on assessed needs and the relevant criteria. Our team can discuss options, while our ITU nurses assess complex health requirements and provide clinical oversight of the agreed care.

Our ITU nurses lead face-to-face, in-house training and regular training in the client’s home, using equipment relevant to their care. Training reflects communication, cognitive and sensory needs alongside any epilepsy, feeding or airway support. Staff follow agreed specialist guidance and individual support plans, with competency assessments and ongoing clinical oversight.

We listen to the person and those who know them well, recognising that changes may communicate discomfort, distress or an unmet need. Our ITU nurses provide assessment and clinical oversight of health concerns, involving appropriate specialists where required. Regular nurse visits and reviews help update care and support plans, including overnight support where assessed and agreed. On-call clinical support is available to the care team for concerns arising between visits, with escalation guided by the individual’s care plan. Risk assessments are reviewed when needs or circumstances change.

Contact our team or complete the online referral form. Share the person’s communication preferences, health needs, current support plans and any relevant health passport or specialist guidance. Our ITU nurses assess suitability, complete risk assessments and plan clinical oversight. We then agree next steps around funding, equipment and preparing a team matched to the person’s needs.
