Brain Injury (ABI/TBI) Care at Home

Specialist, nurse-led care at home for people living with acquired or traumatic brain injury. Personalised support shaped around your assessed needs, rehabilitation goals, everyday routines and independence.

Brain Injury (ABI/TBI), Home Care

Acquired and traumatic brain injuries affect people differently. Individual assessments, with input from experienced ITU nurses where clinically required, help determine the level of care needed at home. Support reflects each person’s abilities, health needs, daily routines and personal goals.

Care may include assistance with mobility, communication, personal care, medication and overnight support. Dedicated care teams receive face-to-face, in-house training tailored to individual needs, with care plans regularly reviewed and adapted as circumstances change.

Thinking and memory

After an acquired or traumatic brain injury, attention, memory, planning or processing information may change. The effects are individual and may be less visible to other people.

  • Remembering appointments or following a sequence of tasks may be harder.
  • Busy settings or several tasks at once can feel demanding.
  • Abilities and support needs vary from person to person.

Independence includes having choice and control, even when you need support.

Care begins

Care begins with regular reviews, updates and communication.

We provide personalised care at home for people living with an acquired brain injury (ABI), including traumatic brain injury (TBI). Support can include personal care, mobility, communication, nutrition epilepsy care and associated complex health needs. Our ITU nurses provide clinical oversight and regular nurse visits, shaping care around the person’s abilities, preferences and rehabilitation goals.

We can work alongside hospital teams, therapists and case managers to plan care at home where assessed needs can be safely met. Our care teams support agreed rehabilitation activities and everyday routines under the direction of the relevant professionals. Our ITU nurses provide clinical oversight, helping maintain safety, comfort and continuity as the person adjusts to life at home. Our ITU nurses complete individual risk assessments covering mobility, cognition, communication and any associated clinical needs.

Depending on circumstances, care may be funded through NHS Continuing Healthcare, Children’s Continuing Care, local authority or joint funding, a Personal Health Budget or private funding. Insurance or compensation arrangements may also be relevant following an injury. Our team can discuss options with families and case managers, while our ITU nurses assess needs and provide clinical oversight.

Our ITU nurses lead face-to-face, in-house training and regular training in the client’s home, tailored to their care plan and equipment. Training reflects needs such as communication difficulties, changes in memory or behaviour, mobility, epilepsy and any airway or feeding support. Competency assessments and ongoing clinical oversight help staff deliver consistent care alongside specialist rehabilitation guidance.

Our ITU nurses assess the person’s clinical needs and provide ongoing clinical oversight through regular visits and reviews. We involve the person, their family and relevant professionals when reviewing routines, rehabilitation goals and support levels, including any changes in epilepsy care needs. Waking-night or 24-hour care can be considered where assessment identifies a need. On-call clinical support is available to the care team for concerns arising between visits, with escalation guided by the individual’s care plan. Risk assessments are reviewed when needs or circumstances change.

It helps to share current assessments, discharge information, rehabilitation goals and any case manager details. Where the person has epilepsy, please also share their seizure care plan and any prescribed rescue medication protocol. Our ITU nurses assess suitability, complete risk assessments and plan clinical oversight with the wider team. Start dates depend on funding, equipment and preparing staff for the person’s needs.
