Commissioning Complex Care
Getting Complex Care Right From the Start
Komplex Care works with ICB and Local Authority commissioners to plan and mobilise nurse-led complex care packages for adults and children in their own homes. Our approach begins with robust clinical assessment, risk planning and needs-led staffing to ensure every package is clinically appropriate, properly resourced and sustainable from the outset. Through specialist recruitment, bespoke training, clear reporting and ongoing review, we support safe discharge, reduce the risk of package breakdown and help individuals remain at home with care that adapts as their needs change.

Care begins
Care begins with regular, reviews, updates and communication.
Komplex Care provides nurse-led complex care at home for adults and children across England. We support long-term care packages, hospital discharge, step-down rehabilitation, planned respite, palliative care and end-of-life care.
Mobilisation timescales depend on clinical complexity, location, staffing, training, equipment and funding approval. Once we receive the referral information, our clinical team assesses suitability and provides clear next steps and an indicative timescale.
We recruit care professionals around the assessed needs of each package. Team members complete specialist training and competency assessments, supported by nurse-led clinical oversight, care-plan reviews and clear escalation procedures.
Subject to clinical assessment, we can support individuals with ventilation, tracheostomy, airway management, enteral feeding, seizure management, acquired brain injury, spinal injury, neurological conditions and other high-acuity or long-term needs.
Commissioners should provide the individual’s clinical needs, current care plan, risk information, location, required hours, funding route and preferred start date. Hospital discharge documentation and relevant professional assessments should also be included where available.
We work collaboratively with commissioners, case managers, hospital teams, healthcare professionals and families throughout assessment, discharge planning and care delivery. Regular communication and reviews help keep clinical decisions, risks, responsibilities and outcomes coordinated across the wider MDT.
